A shoe insert (orthosis) is sometimes recommended to support the arch. The shoe insert (orthotic devices) needs to be worn at all times, in a well fitted and supportive shoe (e.g. sneaker, or similar), for the rest of your life. A shoe insert, no matter how sophisticated or customized,  can neither alter, nor improve, the alignment of the bones & joints in the foot. The Framingham study (14), the largest of its' kind, along with other publications, demonstrate the correlation of poor foot posture (flat foot), with bunion deformity.
